Dear Diplomate,
The form for the re-evaluation of Diplomate
status is available on this website. This form has to be completed by
every Diplomate every 5 years (non-practising diplomates only need to
complete the first 2 parts). For an explanation of the re-evaluation
processes please see Article 4. Section 10 of our constitution.
As agreed during the AGM in September 2005 and in
accordance with the EBVS requirements Diplomates must score more than
100 points over the preceding 5 years in order to be considered ‘practising’.
You are advised, however, to complete the form in full to give a clear
picture of their activities.
We have tried to make the form as clear and easy
to use as possible. Please consult chapter 10 of the current Information
Brochure for further information on completing this form. Please contact
me if you have any questions.
Thank you very much for your understanding and
best regards
